I’m assisting my dad, Constante C. Molina (SSS member), in processing his retirement benefits. We’re currently staying abroad, and we were instructed by SSS office- Laoag City Branch to download the “E-4 FORM” to be filled-out/signed by the applicant. We’ve been searching for it online over the weekend, but was not included under “downloadable forms” on SSS Philippines website.

I would appreciate if you could send a copy of the “E-4 Form” to my e-mail add (anilom_02@yahoo.com) ASAP. We’ve actually started applying for my dad’s “retirement pension” 3 months ago (June 2009), and the processing has been delayed due to some “changes in the system” made recently, as explained by your Laoag Branch office. Nevertheless, we hope for a faster and efficient processing this time.
