    Good day!

    I would like to make a follow up on my brother’s death and funeral claim named JAVIER M. ENANOD with an SSS No. 3401297480 born on December 3, 1980 and died on April 3, 2007 We have completely filed the needed requirements last March 18, 2010.

    Kindly give me an immediate feedback regarding this matter because we are on a financial crisis as of this moment. Moreover, I can’t leave my work every now and then to check this claim to your good office.

    My husband, Joseph Julio M Talabis died June 25, 2010, his SSS #. He had stroke during work and hospitalized. His employer shouldered the hospital bills. Their employer has not remitted a single remittance on any government offices (SSS, Philhealth, Pag-ibig).

    May I request his summary of contributions and benefits.

    If your husband’s office did not remit a single payment to the SSS, Philhealth and Pag-Ibig, then wala po kayong makikitang summary contribution for his latest employer. You should have filed for a case in that matter pero kung may previous employer po ang husband ninyo, yun lamang po ang magrereflect sa contributions nya.
