Are you on your way to apply for a SSS ID card? This short guide will help you in the process of your application from ID form and requirements to the tips on where you can find SSS branch with ID capture machine. The SSS form used for ID application is E-6.
SSS ID Card Application Requirements and Form
With respect to SSS ID capture machines, there is one for you sure you can find at the SSS main office which you can find at SSS Building East Avenue, Diliman Quezon City. For telephone numbers before you apply, please call (632) 920-6401, 920-6446 while you can also try to email them at – member_relations@sss.gov.ph. Makati also had their own ID capture machine as well as Manila branch.
Requirements for Application
Primary documents -
- Passport
- Professional Regulation Commission (PRC) card
- Seaman’s book
If you don’t have any of these primary documents, you may use the following as substitute as follows:
- Driver’s license
- Valid National Bureau of Investigation (NBI) clearance
- School or company ID
- Postal ID
- Senior citizen card
- Major credit card
- Voter’s ID
- Savings account passbook
- Alien certificate of registration
- Government Service Insurance System (GSIS) member’s record
- Certification from the Office of Southern/Northern Cultural Communities or Office of Muslim Affairs
- Taxpayer Identification Number (TIN) card
SSS ID form E-6 may be downloaded at the SSS downloadable forms section on their website. On the other hand, there is an extra charge for lost ID, damaged ID or for removal of the date of birth as per owner request. The usual mailing of the SSS ID is withing 30 days. IF you don’t receive it withing that period, please contact or inquire at your nearest SSS branch.

Pano kung ung company ay hnd ngpprocess ng sss contributions. I mean d sila ngkakaltas s sweldo namin. Ano pong actions dapat gawin. Maraming English On-line Teaching companies ang hindi ngcocomply s Philippine laws. Hindi rin sila ngi-issue ng employment id kaya di nmin masabi n employed kami sa mga transactions na ginagawa namin. Pwede ba mg voluntary member na lang? Thanks.

the machine for i.d cards here in iloilo has been broken and still hasn’t been fixed yet. when can we have our i.d then? it has been broken for more than 2 years now..

Yes it is… you’ll just have to pay the same way you did before but you’ll have to ask the SSS about matters on how much because you are now considered an OFW member… Such cases change when you change status from self-paying to another category which is working member abroad.

i just want to express my frustration with sss…. na-id capture na ko nung january 2008 and nafollow up ko na sa sss and to whomever possible ang tungkol dito but very unfortunately until now 2010 na di pa rin dumarating ang id ko. sabi sa hotline nyo pag nasend na daw ninyo at may job order number na wala na kayong alam pagkatapos. anong klaseng serbisyo ba ang ibinibigay nyo sa aming mga Pilipino? napaka-on time magbayad ng conributions ng company namin pero eto kayo, you’re making us all stupid waiting for this very precious id. and now that i im planning to apply for another baka naman po pagbayarin nyo pa ulit ako na parang ako pa ang may kasalanan.

You will have to file for a SSS change of status. Usually, if you work, you will have to get the SSS change status form from your office so that they also know about the changes on your SSS membership account as this will affect their contribution to your Social Security System remittance. In case you do not know, the system is that if you remit Php 100.00, your company will also remit their part which is of the same amount.

Hindi naman po ganoon. Actually by experience po ay mas madali ang tanghali magpagawa ng SSS ID kasi mabilis naman ang pila doon sa loob sa dami ng counters. Try nyo po para po alam din nyo… Iba iba naman po ang experience ng mga nagpapa SSS ID capture doon e.

I tried to access my SSS account online and check my eligibility for a loan. The reply is that I am NOT qualified for a loan since my status is TEMPORARY. I have my SSS Number but does not have an ID yet.

Hanggang ngayon po sira parin daw ung machine para sa sss id dito sa Lucena City branch. ang tagal ko n pong gustong magpa-id kaya lang itinuturo pa kami sa main branch. hanggang kelan po kaya mgaayos yun, mahirap lang po ako at hindi po madali para sa akin ang pumunta pa main para sa id ko. If your SSS ID was lost, then first step is to go get yourself an affidavit of loss and bring it to the SSS branch where there is ID capture machine. The SSS main branch is best for this because I have learned that all other sub branches have their SSS ID capture machines no longer working.

You can follow up your concern to the SSS office near you or from where you applied for a SSS ID. It is always best to come over in person than to transact via telephone po. Also the delivery address matters a lot so be sure that you gave the exact address you are currently living in if the delivery address is in your residence. If the delivery address you gave is in your office, then double check with your company.

Lahat po ng government institutions and offices ay closed on Saturday and Sundays and that includes the Social Security System or SSS.

We have the same concern po tungkol sa SSS ID application. We also want the SSS to give their members applying for an SSS ID to claim it instead of sending to either employer or residence. This way, ID’s don’t get stuck for long if they are already made. Let us all hope that the SSS hears this or the new government of Benigno Aquino does.

Agapita, yes you may… but you will have to get first an affidavit of loss for your ID and paki pa notarize (notaryo) po itong affidavit ninyong ipagagawa bago pumunta sa alin mang SSS branch na may SSS ID capture machine to apply for a replacement ID.

nagnareplace ako ng bgo id kc nawala ung id ko last november 2009 pa and yet ala pa din…sa main branch pa ako ngapply para sure na mabilis…naifoolow up ko na siya at ang sabi sa customer service..wala daw kayo availble na machine mga sira na daw…how can this be possible??? wala na ba pondo SSS??? san napupunta ang binabayad nmin kada buwan

You may apply for an SSS ID card in any branch near you that has the digital imaging machine. Most SSS branches do not have this anymore of if they have one ay madalas na out of line or out of service. It would be best if you can go to the main SSS branch kasi mas madali po doon at less ang pila dahil maraming counters to entertain application.

Nag file po ako ng Request for additional name sa SSS Fresco last Oct 20, 2009.Sabi sa SSS 45 days ang process pero hangganag sa oras na ito di ko pa nareceived ang status ng application ko.
Pinuntahan ko uli ang SSS pero under process pa din daw mag antay na lang daw ng tawag. Problema baka nag retiro na ko di ko pa rin ma received. Follow up ko lang uli kung kaylan talaga nila matapos ang processing.

may claiming stub na po ako ng id, nag hintay ng 6 months, nag pabalikbalik 7 times sa sss branch quezon city sabi mag hintay,medyo matatagalan daw bat ganon, sabi sa likod ng application form 30 days as in 30 DAYSSSSSS!!!!, mahalaga ang id na to at bawat senti mong hinuhulog namin sa inyo, id lang di nyo pa mabigay ng tama.

ng-apply po ako ng ID replacement last October 2009 and until now wala pa rin po ako natatanggap na ID from SSS. I heard matagal ang processing sa Main Branch sa Diliman, ask ko lang po up to when ako mag-aantay ng mail bago ako mag follow-up sa Diliman? Sobtang hirap po pumila sa SSS at kailangan pa mag-absent sa office. Salamat.

hi sss, lahat na nagawa ko pumunta, sa main branch. nag pabalik balik 7 times, nag hintay sa bahay. tumawag sa phone. gaano ba talaga katagal mag print ng id? 1 pc per hour?. 3o days 30 working days and sabi sa likod ng form. 30 working days!!!!!, sana wag ng i padala,plss. itang na loob. maawana kayo. mag bigay na kayo ng id, plsss.

Hi, nag apply ako ng sss id sa sss binondo manila branch, after ko ma fill up ung E-6 and checked by window D personell dun sa sss, he said punta daw ako sa main office which is nasa quezon city. dito lang ba puede mag pa id system? sa mga nabasa ko dito parang meron sa caloocan which is malapit sa location ko. and what time puede pumunta doon? some says 4am dapat nakapila na?

I,ve been waiting for my ID to be released it,s been 6mos already I filed it in Manila I want It to be transfered to my employer’s address in RJT Hollowblocks and Gen. Merchandise as early as possible,and I’ve been complaining about my name to be corrected as Salvador S.hulleza,Jr.and not Salvador S.Nulleza,Jr.I already fill-up E-4 3 times already.And I applied for a loan and was’t able to get my check.I am waiting for my loan to issue with me. Im going nuts over the process on how to apply ID card. I went to Las Piñas to but was told to go to the main office in East Avenue. Fine, I went there but was then again advice to go to the Las Piñas branch since I registered there. So which is which… I just wanna let these people know that people going there needs honest and efficient answer. We are not a ball that you can throw anywhere just because you said so.

I have requested Nov. 26, 2009 for an ID with SSS but its been more than a year compared to 30 days grace period of receiving the ID… Until last week, I have followed up with SSS Naga but they implied that the ID is STILL FOR PRINTING… Its been 10 months that my ID is being printed since my last query last February this year…. Mas mabilis pa ata printer kong Epson LX300++, it can print more than 50 pages per hour, printer ng SSS 10 months na d pa makaprint.

I am calling your branch.. and “hirap ng tumawag” tas pag pinick-up na ang phone “iHAHANG lang”… Para mainis ka at bababa mo yung phone at di na sila makontak ulet…!!! grabe.. ganyan ba talaga ang staff sa lahat ng branches ng SSS? I would like to follow up the delivery of my ID to my home address – which has been indicated as to where it will be delivered on the application form. It has been filed on August 19, 2010. According to your employee (didn’t catch the name), it should be delivered 30 days after the application process is complete. Unfortunately, I haven’t received it yet. I need my ID for a valid identification since I changed my status to my married name.

kumuha ako replacement sa sss id nung March 23, 2010 pa hanggan ngayon wala pa din..
yun po bang status na nasa online inquiry updated po ba kasi nakalagay sa akin “ID Captured on process” abah mag 1 year na ah..
isa pa pong tanong..pano po pala pag lumipat ng address pwede kaya mag request na i change kung saan nila ipadala..

ganito lang po kasimple anglahat ng problema nyo… magtatrabaho tayo tapos magbabayad sa sss. nanakawin nila ang pera natin… tapos bili sila ng sasakyan.. yung mga empleyado naman nila, kala mo kung sinong mga matatalino.. alam naman nila na ang pinaghirapan natin, pinapangsahod sakanila… pinapakain sa mga anak nila at lahat pa yan nakaprivate… ang kakapal nyo!!! ang maganda dun, iilan lang ang pang capture nila ng sss id, pero tingnan mo mga bahay ng mga yan lalo na mga officials… mahiya naman kayo… magtrabaho tayo ng pareho!!

SSS…ILOILO…ang susungit ng mga empleyado…they are public servant pero kung makaasta akala mo san sila kumukuha ng pansahod nila eh sa mga taong nagreremit sa kanila at member nila pero kung ituring nila yung mga member daig pa mga bobo…akala mo kung sino sila???dapat pagtu unan naman to ni PNOY ng pansin…tapos may kanya kanya silang procedure binibigay…walang bang over all prosedure?magtanong ka ang susungit pa!bakit magtatanong ba jan yung mga member kung alam nila?yung forms kaw pa magpapaxerox…yung remittance namin saan pumupunta…hopefully makita to ng pamahalaan at mabigyan pansin…tanx

hello po. ask ko lng anong form ang dapat kong i-fill-up kapag pupunta ako sa sss branch para mag hulog. forst tyime kong maghuhulog pero matagal na kong my sariling SSS number kaso hindi ko pa sya nahuhulugan kahit isa.wala din akong work at gusto ko lng maghulog na. anong form ng sss ang dapat kong hingin at ilan dapat ang ihuhulog ko pero volunteer lng. tnx.

Last april 24, 2010 nagfile po ako ng SSS ID (E-6) pero until now wala akong natatanggap na ID. Tama naman un mailing address na inilagay ko dun.. Ganun ba talaga katagal ang pagprocess ng renewal SSS ID???? Sa main building pa ako nagfile kahit malayo at buntis ako pumila ako para lang magkaroon ng ID kc need sa Maternity ko, ngaun malaki na anak ko wala pa din. nakakalungkot lang isipin na ganun kabagal ang pagprocess maging ang mga nagtatrabaho dito……. Bakit ang tagal nang process sa SSS ID pag nawala..nag file ako nang affidavit last year. mag 1 year na next month sa pag process ko kailangan ko na ang ID ko for requirements. Ganito ba talaga ang process nang ID katagal. I hope that may run na pong feedback ito. Nung nag pa ID ako ang dali lang 3 months. natanggap ko… I hope lang sana mapadala na nila kailangan ko lang for valid identification ko… thanks

Hi! I would to ask about my SSS Employee Static Information. Saan ko po ba makukuha yun? I tried to register online but it was so frustrating that only Member and Employer could have an account. Ask ko lng kung how long will it take para ma-fix ng SSS Cebu yung ID capture machine nila.
I went there last January and was informed na sira daw yung ID capture nila and was advised to return after a few days or have my SSS ID processed sa Lapu-lapu branch.
Then, last Tuesday (April 17, 2011) , ngpunta ulit ako kaso not working pa rin daw yung ID capture.
Please give an update. Thanks.

I applied for a new SSS ID Card first week of Sept. last year in Naga City branch but until now I don’t receive it… I wonder why it takes so long for you to process the said ID Card while in other government agencies only about 15 minutes you can have your ID card delivered even the renewal of the driving license it takes about 1 hour at the most. Kindly explain the reasons why despite of your announcement that only 30 days almost 1 year now no ID card on sight.

My brother, Demetrio O. Grafia, died 3 years ago and had not made any claim with the SSS such as burials, pension benefits or what not. How would I proceed to do this? Mylene, SSS IDs are sent in your home address unless you are employed and you opted in for the SSS to send your ID in your office address or office PO Box. For now, the SSS doesn’t manufacture SSS IDs yet. It wasn’t since October 2009 so all IDs are on pending status since.

Mr. Leo, since 2009 October pa po nag stop na ng manufacturing ng plastic card ID ang SSS kaya up to now wala pong nabibigyan ng ID. Kung kailangan nyo po, you will have to request for a temporary SSS ID by going to the nearest branch and asking for more information.

Ms Wilma, since 2009 po di na nagmamanufacture ng plastic ID ang SSS or stop making po sila kaya hindi lamang po kayo ang nag aantay. Yung iba po ay since 2009 pa po di nakakakuha ng SSS ID. Until they resume making ay wala pong mabibigyan, puro po application lamang and inaaccept nila.

Mr Sam, since October 2009 pa po nag stop ng manufacturing ng SSS ID po. Nagkaproblema po sa contractor ng plastic ID kaya po lahat ng taong nag apply since October 2009 ay di pa po nakakuha ng SSS ID nila.

Greetings!
About an hour ago, I came from the Social Security System (SSS) Office at your branch located at the G/F SSS Makati Building, 6782 Ayala Avenue corner V. A. Rufino Street, Makati City. And I am not happy.
I wanted to replace my lost SSS Identification Card and I was impressed how efficient SSS was, on using its website to disseminate information. I downloaded the forms I needed and followed the instructions on the form. Brought with me the required forms to the SSS Branch Office and started to queue. I was online for about 30 minutes. The line was very organized and the Securtity Guards were very friendly.
When my time came to be served, the gentleman on the counter, who was very polite, informed me that I do not have the complete documents. He further informed me that SSS is in the process of upgrading its ID System and as such, the system changed from middle initials to full middle names. This information was not in your website and I was upset because this entails that I have to apply for a birth certificate and I have to return back again. Not to mention the time it will take! If you are on my shoes, I am sure you will also get upset.

Since you provide world-class services to your members (that’s your vision, by the way), there should be an advise somewhere that SSS is updating its system, explaining that the old system only carried middle initials and to change it to middle names, one should bring a birth certificate from the National Statistical Office. Furthermore, it is really amazing how illogical for a government agency not to honor a public document. I had my passport, which shows my middle name, but SSS does not honor If you do not honor the passport. If you do not honor the passport, then why do you put in your website that SSS Form E-6 must be submitted together with any of the following primary documents – one of them is the Passport. I find this very bureaucratic.
Even the picture of the SSS ID in your website is an old one!
And one thing more, to avoid people signing the forms with blue ballpoint pen (like what I did), perhaps you should consider putting the word “Use Black Ink” as a watermark across the form.
I feel a victim of your system. I can not do anything but to follow your system. I am at your mercy. I hope you are happy!

I have applied for an SSS ID last December 2009 knowing it would somehow be not any longer than 3 months to get it. Kasi naman po, the banks don’t accept transactions na po without 2 vaid IDs and signature on that said ID, which means by the way, na di po nila tinatanggap ang nbi clearance or marriage certificate to prove your identity with your signature. August 2011 na po, wala pa rin po akong natatanggap na SSS ID. I just recently went to SSS to inquire about this matter. Simple lang sagot nila. Hintayin nyo na lang po kac po latest na nareceive po ng office namin is August 2009 pa po…

I applied for my sss id last june 2011, it’s been 3months and i still don’t have the ID yet.SSS doesn’t even give a specific date to when it will be release or atleast estimated time of arrival of the ID..

hi,, ang tagal ng sss id ko, since feb.2010 until now wla pa rin. kailan ko ma recieve ang sss id ko? im here in dumaguete city negros oriental.

Ask ko lang kung may pag-asa bang makuha ang mga ID? Hindi rin pala magagamit eh bakit kinukuhaan nyo pa kami ng ID? Tanggalin nyo na lang yung mga staff doing the ID’s para mabawasan ang mga binabayaran namin ng walang dahilan. Kahit kayong mga empleyado hindi nyo alam kung kelan ma-rerelease and mga ID namin. Wala man lang kayong target date. Nasa computer naman lahat at may machine na rin to print the IDs pero parang bakit walang epekto. Pilipinas kelana ka nga ba aasenso kung ahensiyang tulad nito ay walang pakiaalam sa taong nagpapasoh sa kanila?

kelan po ba maging availabale talaga yung sss digitized id card ko,for change status & name lang po yun, last 18 february of last year pa po kasi yun?..patatawagin nyo sa nyo sa hotline nyo sa local 5711, then guard nyoa lang sasagot.gaanu ba kalawak ang alam nila sa mga inquiries ng tatawag? di ba dapat sss employee ang mag received ng mga calls?at pag sa counter ka naman mag inquire, sasabihin nila, tumawag ka sa hotline, pag katapos ko na hintayin yung haba ng counter #..ang susungit pa naman ng mga employee nyo, ang hirap i- approach..mag bigay naman kayo ng definite date kung kailan nyo ba talaga ma- release yung digitized id ..

Dear Ma’am/Sir,
I applied for SSS ID last Nov. 2009 and I’ve heard that your office just stopped in producing ID. Actually, I already followed up last year, in SSS-Diliman, where I applied my ID, and they told me that they will resume producing ID this year. Why is it that my officemate who applied only last year, got her ID already while I am still waiting for my ID that was applied ahead of her application?

It’s been a year since I applied for an ID at SSS Main in Quezon City and up to this time, I haven’t received it. I was told that it would take only 3 months. Please be sensitive enough to all taxpayers like me who are paying your salaries

Last May 2010 ko pa po inaaply un and 6-8months lng daw po un pero 1 and a half year na po wala pa ata,.,. gnun po pa tlga system ng sss???,.. mukhang tlga pinanangatawan nyu po na bastah mga government employee kung hindi corrupt e tamad,… pxenxa na po sa mga sinabi ko pero parang ndi po kasi justifiable yung work po ng mga tagaSSS,… I hope maiicip nyu po etong mga sinabi ko,…maraming salamat po,.

I applied for my SSS I.D last May 2010 and until now October 2011 na wala p din.. nung nag tanong ako dun sabi 2-3 months pero til nw wla pa din po.

ask ko lang poh.. kung need ba namin mag apply ng bagong I.D..? kasi iba na din yung address namin.. and dumating na yung I.D ng mom ko, ang ganda kasi sama-sama na lahat.. as GSIS, PHILHEALTH, PAG-IBIG, and SSS I.D

Please help me with my concern. I had my SSS Id application in Cebu with my previous employer. The problem is that SSS already mailed my ID to my previous employer but our company was dissolved already early this year. I am now in another company and they need my SSS ID as my requirement for regularization. I asked my brother to see somebody in SSS Cebu to claim for it but they said its in the post office. He went to see Cebu Post office but they said they returned it to SSS. I do not know what to do. please help me with this. thank you so much

paanu po ang gagawin namin kumuha kami ng new id ng mister o pero pagdating main office ng sss binigyan lang kami ng skedule ng pagbalik para daw sa picture captured ang problema 3 months pa….ang skedule namin…after that time syempre pareho kaming busy nawala na sa isip namin yong sss…so anung dapat gawin namin ang layo namin sa main office ng sss..sobrang pahirap po ang ginagawa nyo sa mga tao.

at sana po kung maari maglagay kayo ng branches na pwde sila mag isyu ng new id ng sss…kung lahat ng tao pupunta sa main office at pababalikin nyo lang aba di na po mura ang pamasahe yong iba galing pang province di ba kayo naawa.sana ibalik na lang ulit sa id card yong yellow kahit papanu yong release agad.kung kelan nag improve yong id syang bagal at pangit ng patakaran….gawaan nyo ng paraan yan sayang yong malaking pera ng sss pinapasok sa sss kung ganyan ang gawain naman …di kayo nakaktulong sa bayan
