There are different SSS claims and with each of them attached is a specific SSS claims form available for download at the official website of the Philippine Social Security Service.

As an additional instruction on how to download those, please go to www.sss.gov.ph and you will find below the lower left corner a link to where you can find all the different SSS downloadable forms.

Click that link and you will be led to the page where you can see all the PDF file format of the downloadable forms. That means that you can opt in to save the download in your computer for future use or printing.

  • Claim for Death Benefit (DDR-1)
  • Claim for Disability Benefit (DDR-1)
  • Claim for Funeral Benefit (BPN-103)
  • Claim for Retirement Benefit (DDR-1)
  • Claim for Maternity Benefit (Mat-1)(Mat-2)
  • Claim for Sickness Benefit (CLD-9N)(CLD-9A)
  • EC Claim for Temporary Total Disability or Sickness (B-304)
  • EC Claim for Temporary Total Disability or Sickness (B-300)
  • EC Claim for Temporary Total Disability or Sickness (B-309)

Remember that saving the forms will only take you a few minutes and will involve the same process just like saving an ordinary document. I have divided all the forms into different categories so others will not haveĀ  a hard time trying to look for what they just need.     Good day!

    I would like to make a follow up on my brother’s death and funeral claim named JAVIER M. ENANOD with an SSS No. 3401297480 born on December 3, 1980 and died on April 3, 2007 We have completely filed the needed requirements last March 18, 2010.

    Kindly give me an immediate feedback regarding this matter because we are on a financial crisis as of this moment. Moreover, I can’t leave my work every now and then to check this claim to your good office.
