There are different SSS claims and with each of them attached is a specific SSS claims form available for download at the official website of the Philippine Social Security Service.
As an additional instruction on how to download those, please go to www.sss.gov.ph and you will find below the lower left corner a link to where you can find all the different SSS downloadable forms.
Click that link and you will be led to the page where you can see all the PDF file format of the downloadable forms. That means that you can opt in to save the download in your computer for future use or printing.

Remember that saving the forms will only take you a few minutes and will involve the same process just like saving an ordinary document. I have divided all the forms into different categories so others will not have a hard time trying to look for what they just need. I would like to make a follow up on my brother’s death and funeral claim named JAVIER M. ENANOD with an SSS No. 3401297480 born on December 3, 1980 and died on April 3, 2007 We have completely filed the needed requirements last March 18, 2010.
Kindly give me an immediate feedback regarding this matter because we are on a financial crisis as of this moment. Moreover, I can’t leave my work every now and then to check this claim to your good office.

My husband, Joseph Julio M Talabis died June 25, 2010, his SSS #. He had stroke during work and hospitalized. His employer shouldered the hospital bills. Their employer has not remitted a single remittance on any government offices (SSS, Philhealth, Pag-ibig).
May I request his summary of contributions and benefits.

If your husband’s office did not remit a single payment to the SSS, Philhealth and Pag-Ibig, then wala po kayong makikitang summary contribution for his latest employer. You should have filed for a case in that matter pero kung may previous employer po ang husband ninyo, yun lamang po ang magrereflect sa contributions nya.

My sister’s husband died July 15, 2010. Until when can she claim for death/funeral benefits for her husband. Is there a time limit she can claim?

My father died october of 2000, by then i was 10 yrs old. my mom claimed for death benefits but we did not start remittance.
My question:
1. now that i am 21 yrs old could i still claim the death benefits for the years that we did not get any pension?

I would like to know where to follow up for our application on regards to death benefits.. our claim application was rejected due to the discrepancy on the name of my brother. with this, We submitted the duly required affidavit for proof. I want to know now the whereabouts of our application. we, by the way, applied and submitted the requirements at San Francisco branch..

Hi i would like to ask lang po kung para saan ung Guarantors form na needed to claim death claim. and also about changed in guardianship kasi my sister is still minor and she has pension from the death benefit of my dad. now the needed forms should be notarized before submitting but the notary office would not like to due to my mom being a second wife but has an original marriage contract from NSO. What im doing is just change in guardianship. ano po ba dapat kong gawin ngaun?

just want to know if the survival spouse has an additional pension as she reach the age of 75.my mom was 75 yrs old last may 31.can she claim an additional pension on that matter? as what other pensioner said my mom can claim it. where and when?tnx a lot.

3 months npo akong pregnat ngayon, kya lng hndi pa po ako nkapag file ng MAT1 ko kc po yung last na voluntary contribution ko is nung march 2008 pa since po nun hindi nko nkapag hulog ulit, besides meron parin po akong loan na once ko lng din po nahulugan. paano po kaya iyon? makakapag file parin po ba ako ng maternety ko khit na hindi n ako nakakapag hulog ng monthly?
