SSS ID Card Requirements
Are you on your way to apply for a SSS ID card? This short guide will help you in the process of your application from ID form and requirements to the tips on where you can find SSS branch with ID capture machine. The SSS form used for ID application is E-6.
SSS ID Card Application Requirements and Form
With respect to SSS ID capture machines, there is one for you sure you can find at the SSS main office which you can find at SSS Building East Avenue, Diliman Quezon City. For telephone numbers before you apply, please call (632) 920-6401, 920-6446 while you can also try to email them at – member_relations@sss.gov.ph. Makati also had their own ID capture machine as well as Manila branch.
Requirements for Application
Primary documents –
- Passport
- Professional Regulation Commission (PRC) card
- Seaman’s book
If you don’t have any of these primary documents, you may use the following as substitute as follows:
- Driver’s license
- Valid National Bureau of Investigation (NBI) clearance
- School or company ID
- Postal ID
- Senior citizen card
- Major credit card
- Voter’s ID
- Savings account passbook
- Alien certificate of registration
- Government Service Insurance System (GSIS) member’s record
- Certification from the Office of Southern/Northern Cultural Communities or Office of Muslim Affairs
- Taxpayer Identification Number (TIN) card
SSS ID form E-6 may be downloaded at the SSS downloadable forms section on their website. On the other hand, there is an extra charge for lost ID, damaged ID or for removal of the date of birth as per owner request. The usual mailing of the SSS ID is withing 30 days. IF you don’t receive it withing that period, please contact or inquire at your nearest SSS branch.

I paid 300 pesos for a new ID and I applied last April 2011 and just last month I inquired again and the in-charge told me. He does not have my ID yet. So, I think It takes years. I have regrets I should not have change my old ID to a new one.. but I have no choice because I need an ID with a new surname.

ive lost my sss id and i want to have a new one. how can i apply for a new id especially like me ofw. is there any other option.
SSS is one of the worst services we have here in our country.. requesting for things like ID, contributions and such will have you wait for months.. even years.. and they’re very up-to-date with deducting us with our contributions.. and once you will go to one of their offices, sila pa yung yamot na haharap satin.. wtf diba? very poor service, very poor facilities and napaka walang kwentang ahensya.. pati website nila walang silbe.. mag reregister ka online, wala din naman pinupuntahan yung links.. wala ka din marereceive na confirmation or even an error message.. napaka tanga ng gumawa ng website ng SSS.. sobrang katangahan.. masama pa nyan, tayo pa magdudusa sa sobrang katangahan nila.. hay.. sana lang may magawa ang gobyerno dito..

mtagal na ko nag aply ng sss id last jan. 2010 pa. until now wala p rin!!!!! mas ok ba? mag aply ulit ng sss id!!!
Follow up ko po yung SSS ID ko dahil until now hindi ko pa rin alam kaylan ba magiging available yung ID ko last July 2011 ko pa iyon na process sa Diliman brand (Main Office) need ko na talaga kasi yung ID ko. paki email naman sakin ASAP talaga. Thank you po.

i’ve been trying to call sss fti branch on their hotline but its keep on busy.. may i know if i can check my sss id through email if its already available?. i filed a new sss id last september last year but im no longer staying to the mailing address which i wrote to my application. what im worried is its not easy to do another transaction with sss. please give some info regarding this matter so i will know what will i do.. tnx god bless sss.

ask ko lang po… pede po ba ako mag apply ng E-6 SSS Card po kahit wala pa akung hulog… self employed po kc ako eh! like ko lang po kcmag ka i.d ng SSS para po din sa PASSPORT. wala akung maipakitang supporting id. nid din po kc ng SSS id card ngaun sa passport…

wow!open ko itong website na ito kukuha sana ako ng info tungkol sa SSS ID,puro palpak parin pala dyan sa SSS,mukhang aabutin din ako ng 3 years pala bago makuha ID…ano naba ito Pilipinas!!!!!!!!!!!!!

I applied for a new sss id last April 2010, but until now I don’t have my id yet. One of the staff answered me that it still underprocessed. It’s been two years!
nakakainis tawagan ang sss,lahat ng number na na-dial ko walang sumasagot,even sa hotline number..sana hindi na lang kayo nagpalagay ng telephone kung hindi kayo macontact!
ang tagal ko n naghuhulog s SSS, nagyun lang ako magloloan..tapos nlaman ko s TEMPORARY ang number ko s SSS.Ang dami hiningi requirements.Napasa naman ako ng colored photo copy ng NSO ko kasama p ung original copy ng NSO,sasabihin nila malabo at ayaw nyo tanggapin.My two valid id’s na.Tapos hiningi BAPTISMAL certificate.ano na nman b kalokohan yan? pera ko niloloan ko ang hirap p ibigay.Hindi man lang inaupdate s mga tao kung ung number nila sa SSS eh TEMPORART OR NOT.Kaya nahihirapan mag loan agad dahil sa MALI SISTEMA NYO!! PAKISAGOT NGA?

Me and wife applied for the new id (sss) since August 2011,but until now both of us didn’t received yet our SSS ID Cards.We followed-up in our SSS Branch (Alaminos City,Pangasinan 2404) and also from Dagupan City,Pangasinan),they told just wait for one month.So,please help us to provide our IDs at the earliest.I coming back again from vacation (abroad) this forth coming of 25 September 2012 hoping that our IDs will be given to us from our Post Office thru via airmail.Thanks and your utmost concerned int his regards is very highly appreciated.

Good Day! Gusto ko lng po sana malaman kung kailan ma-release yung SSS ID ko? Nung July 05, 2011 nag apply ako ng panibagong ID kasi nawala, meron ako affidavit of loss. Binayaran ko rin ang panibagong ID. Hanggang ngayon wala pa rin, one year & 3 months na. Kelan kaya ma-release and bagong ID….
